If you are in a hospital and have been told that you are ready to leave but feel that you still require assistance, you may be able to access up to 12 weeks of government-subsidised transition care to support your recovery and help you regain your confidence and independence.
The care and services you receive will be tailored to your needs and can be provided in your own home by our home support service or in Alwyndor’s residential accommodation.
Depending on your needs and goals, transition care can include:
- Personal care, e.g. assistance with showering, dressing and eating.
- Nursing support, e.g. help with medication and wound care.
- Therapy services, e.g. physiotherapy, occupational therapy, podiatry and speech therapy.
Accessing transition care
If you would like to receive transition care after leaving the hospital, you will need to be assessed by the Aged Care Assessment Team (ACAT). You can ask the hospital staff to arrange for an assessor to visit you in the hospital, or you can call My Aged Care on 1800 200 422 to organise the assessment yourself.
Once you’ve been approved for a government-subsidised transition care package, Alwyndor’s friendly staff will manage your transfer from the hospital and ensure you have all the necessary care to support your recovery.
Cost of transition care
The cost of your care will largely be covered by government funding, and you may be asked to contribute to your care as well. Alwyndor is committed to ensuring that members of our community can access high-quality restorative care when needed, even where you are unable to contribute the full cost of your care. We will work with you to ensure you contribute only what you can afford.
